Latest Community news & jobs Age UK are looking for an external evaluator to review their pilot scheme (Published: 21 April 21) Lewisham Links was a social prescribing pilot project based in University Hospital Lewisham (UHL) and delivered by Age UK Lewisham and Southwark. The pilot ran from January 2020 until March 2021. The project scope has been flexible and responsive due to the impact of COVID-19 on secondary care and the local community. The project supported residents in Lewisham who had been admitted to UHL, with the aim of helping to reduce future attendance to hospital by working with the clients to identify relevant sources of community support and to help residents to access these. The project was managed by Age UK Lewisham and Southwark in partnership with Lewisham Heath and Care Partners and UHL. Proposal Lewisham Health and Care Partners and Age UK L&S would like to receive a review of the pilot scheme, to be completed by the end of May 2021. Due to the evolving scope of the project and the challenges it faced as a result of COVID-19, there will be a focus on qualitative rather than quantitative data. They will offer up to £3,000 for a review of the pilot including recommendations for future social prescribing projects in secondary care settings.
